Does he himself say he's not reaching orgasm? Do you believe he can ejaculate when he orgasms? Does he claim he can orgasm, and if so, how?
As I got older I encountered men my age who no longer came, or came very little, but they still had orgasms. In most cases it was was due to prostate issues, some having had infections over the years, or cancer treatments. And of course removing the prostate can really dry things up, except for small amounts from other sources like the Cowpers gland.
Another problem I had was when I was given anti-depressants as part the VA's treatment for my epilepsy. Apparently about 60% of epileptics have depression, so they just gave me this stuff, and I tried just about everything currently available, because I didn't like any of them, kept asking the docs switch to something different without as many side effects.
The varying results on my sex life were curious. I remember some meds wouldn't let me get hard, but I could still have a weak orgasm and sort of dribble-cum. One in particular let me get rock hard, but then I couldn't reach orgasm, no matter how hard & long I tried.
Now that was very frustrating, and they'd tell me these negative effects "should" wear off with time, but they really didn't after months. Maybe your friend is taking something like that.
BTW, I finally told the VA docs they could keep their meds, I didn't think I was depressed anyway. And as I said to them, I can't think of a better way to MAKE a man depressed than to make him impotent. LOL!
