    I see international wires all the time. What I usually see in the instructions are:

    Name of Bank
    Address of Bank
    Name of Account Holder
    Account Number
    Routing Code
    SWIFT Code

    North American banking is SOOOOO antiquated!

    A German client send money from Germany to their BOA account and it converts once it hits. They don't use an intermediary bank. I think BOA is big enough not to need it. It might be your bank's requirement. I've seen that once where there was an intermediary bank was required because the small, local bank would not or could not accept international wires.
