Before I say anything I'm going to suggest if you haven't already to have it checked to make sure all is in place.
If all is in place I would def. start with an ample warm up, the shoulder joint is made up of a very complex group of tendons and bones and can easily be injured. I suggest to not start with dumbbell presses or military presses or any presses for that matter as your first exercise.
I say start with some light lat pulldowns say 2 sets of 40 reps then proceed to some very light 3 sets of 40 reps or so of dumbbell side laterals, and if need be some light upright rows before proceeding to any pressing movements.
As far as macros for your bulking phase, that depends. What is your age? Do you have a fast metabolism? Weight? Height? Etc
