People do kill themselves using weights every year
On Liveleak there is a video of a guy who drops a massively weighted barbell on his chest and crushes it completely, he later died. His two spotters weren't paying attention. I don't recommend you watch it, it is pretty nasty to watch. -

There were some pretty gruesome injuries during the Olympics this year. Even experts can fuck up royally from time to time. -
